CenterWell Home Health is seeking a Licensed Physical Therapy Assistant to deliver in-home physical therapy services, following the plan of the supervising Physical Therapist. You will work in patients' homes, provide therapeutic exercises, document care, and participate in team in-services to support patient recovery.

Requires active PT license, driver’s license, CPR, and strong collaboration with healthcare professionals.

Formación

• Current state license to practice Physical Therapy.
• Valid driver’s license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric changes (physical, emotional, social, biological).
• Read, write, and speak fluent English.

Responsabilidades

• Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment failures, lack of supplies, or staff or client changes.
• Apply infection control and universal precautions in performing patient care activities.
