Job summary

AccentCare seeks a Physical Therapist Assistant for our Home Health team in Burnsville, MN. This full-time role offers a $5,000 bonus and a schedule of M-F, 8am-5pm, with pay range $63,400–$70,400 per year, paid per point basis. Travel to Bloomington and surrounding areas is required.

Under supervision of a licensed Physical Therapist, you’ll treat patients, document care, and educate families while collaborating with an interdisciplinary team to support patient goals and safety at home.

Qualifications

• Associates degree from an accredited school of Physical Therapy.
• Valid license to practice in state of operation.
• Must possess and maintain CPR certification.
• Must be a licensed driver able to travel to all locations.
• Ability to educate patients and families on home safety and medical equipment.
• Ability to implement care plans and communicate findings to PT/physician.

Responsibilities

• Provides patient care and documents treatment per physician orders and patient needs.
• Documents interventions, approaches, and treatment priorities.
• Educates and counsels patients and families.
• Presents patient information to supervising therapist and informs the team of status.
• Provides skilled treatment within agency and payer parameters.
• Collaborates with team on treatment planning and execution.
• Completes daily notes and required forms timely to prevent interruptions.
• Meets productivity standards and participates in meetings.
