Job summary

AccentCare, Inc. in Burnsville, MN is seeking a Physical Therapist to evaluate and treat patients as part of an interdisciplinary team. You will educate patients, document care, and follow current standards of practice; a comprehensive sign-on bonus and competitive salary are offered.

The role is full-time, on-site with a schedule of M-F 8am-5pm. Requirements include CAPTE-accredited PT education, MN licensure, CPR, and willingness to travel to nearby locations.

Qualifications

• Master's degree in physical therapy approved by CAPTE accredited program.
• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.
• Licensed to practice as a physical therapist within the state of operation.
• Must possess and maintain CPR certification.
• Must be a licensed driver who can travel to all business locations.

Responsibilities

• Provides patient care and documents treatment per physician orders and policies.
• Performs patient evaluations and selects appropriate diagnostic materials.
• Informs the physician of results and recommended interventions before initiating care.
• Develops and documents treatment plans with priorities and goals.

What You Need to Know

The Physical Therapist is responsible for providing evaluation and treatment to patients, functioning as a member of the interdisciplinary team, providing patient and family education and counseling regarding concerns, planning, implementing and modifying treatment procedures, maintaining appropriate documentation according to policies and procedures and current standards of practice. This position manages a caseload in accordance with agency productivity standards, attends and participates in patient and other conferences and agency in-services as assigned.

Responsibilities:

As a Physical Therapist, you will:

• Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
• Performs patient evaluations in accordance with agency policy and procedure. Selects appropriate diagnostic material to assess all aspects of patient care.
• Informs the physician of the results of the assessment and recommended clinical interventions prior to initiating care.
• Develops and documents appropriate treatment plans for patients including selecting appropriate intervention approaches, strategies, materials and establishing treatment priorities.
• Provides an organized and efficient patient treatment which includes advising, educating and counseling the patient.
• Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.
• Provides appropriate treatment to patients according to patient needs and within the parameters set by the agency and payer.
• Determines appropriate goals, which are objective and measurable in nature, in collaboration with team members and patients.
• Meets agency productivity standards and uses work hours efficiently.
