Job summary

BAYADA Home Health Care in Folcroft, PA is seeking a Home Health Physical Therapist to perform admissions home health visits and Start of Care assessments. The role requires OASIS SOCs experience and an active NJ PT license, with at least one year of PT experience.

You will work with a multidisciplinary team to deliver in-home rehabilitation, educate clients and families, and maintain thorough documentation using an EMR system. This full-time position offers comprehensive BAYADA benefits.

Qualifications

• Current NJ PT license is required.
• Minimum of 1 year of recent PT experience.
• Graduation from an APTA/CAAHEA-accredited program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated territories for Start of Care admissions.
• Perform diagnostic tests and measurements (mobility, gait, strength, balance).
• Implement wound care interventions using evidence-based dressings.
• Develop and update individualized care plans with PT interventions.
• Collaborate with nursing on multidisciplinary care plans.
• Educate clients and families on rehab activities for health and independence.
• Accurately document observations and interventions in the EMR.
