Job summary

Big Bend Hospice is seeking a Home Health Physical Therapist to deliver skilled PT at patients' homes in Tallahassee. You will assess, treat, and educate to improve mobility and independence in a patient-centered, interdisciplinary team.

Responsibilities include supervision of therapy staff, adherence to Medicare requirements, and ongoing documentation. A Florida PT license and CPR/BLS are required; driving is essential.

Qualifications

Graduate of an accredited Physical Therapy program with Florida licensure.
Experience in home health settings and Oasis documentation is preferred.
Current CPR/BLS certification and valid Florida driver's license.

Responsibilities

Conduct initial evaluations of home-based patients and assess mobility and safety.
Develop and implement individualized treatment plans and functional goals.
Provide skilled PT interventions: exercises, gait training, balance, transfers.
Educate patients and caregivers on home programs and safety measures.
Document evaluations, treatments, progress notes, and care coordination.
Supervise PTAs and ensure compliance with Medicare and state requirements.
