Job summary

Millennium Physician Group in Collier County, FL is seeking a Physical Therapist to join our Home Health Team. You will evaluate patients in their homes, develop individualized treatment plans, and provide interventions to improve mobility, strength, balance, and functional independence.

You will collaborate with an interdisciplinary care team, educate patients and families on rehabilitation and injury prevention, maintain clinical documentation, and ensure regulatory compliance to deliver

Qualifications

• Florida PT license required.
• Minimum 1 year of clinical experience; home health preferred.
• Strong clinical, communication and interpersonal skills.
• Patient-centered approach and reliability.
• Reliable transportation to visit patients' homes.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Provide therapeutic interventions including manual therapy, exercises, and gait training.
• Educate patients and families on home exercise programs and injury prevention.
• Collaborate with physicians, nurses, and other health professionals to plan care.
• Maintain accurate documentation of evaluations, treatment plans, progress notes, and discharge summaries.
• Ensure compliance with HIPAA and Medicare regulations.
