Job summary

Optum's Assured Home Health in Moses Lake is hiring a Physical Therapist to assess and treat patients in home health settings across Washington. You will develop and implement PT plans of care, improve mobility, and monitor patient progress in collaboration with physicians and nurses.

The role requires WA PT licensure, CPR, and reliable transportation; a $10K sign-on bonus is available for qualified full-time candidates.

Qualifications

Current Physical Therapy licensure in the state of Washington.
Current CPR certification.
Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

Evaluates the patient's functional status and physical therapy needs; assists the physician in the plan of care.
Provides services within the scope of practice defined by state laws and the plan of care.
Observes and reports patient response to treatment or changes in condition to the supervising nurse/physician.
Treats patients to relieve pain, restore function, and maintain maximum functioning.
Instructs patient, family/caregivers and care team about the plan of care and progress.
Visits patients per Plan of Treatment and submits progress notes per policy.
Completes evaluations and develops PT plan of care within state guidelines; reports outcomes.
