Job summary

Assured Home Health at Moses Lake, part of the LHC Group within the Optum family, is hiring a Physical Therapist in Home Health. You will assess patients, develop treatment plans, and deliver interventions to improve mobility and function in a home care setting.

This full-time position offers a $10K sign-on bonus for qualified candidates, requires WA PT licensure and CPR, a valid driver's license, and the ability to work with diverse patients across the community.

Qualifications

• Current Physical Therapy license in Washington.
• Current CPR certification.
• Valid driver's license and access to reliable transportation.
• Ability to serve patients of diverse backgrounds.

Responsibilities

• Evaluates patient's functional status and assists physician in care planning.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient responses to treatment and condition changes.
• Treats to relieve pain, restore function, and maintain maximum functioning.
• Educates patient and family about plan of care and progress.
• Visits patients, documents progress, and submits notes per policy.
• Develops PT plan of care within state guidelines.
• Reports outcomes and expected care frequency.
