Job summary

Assured Home Health at Moses Lake, part of the LHC Group and Optum family, is hiring a Physical Therapist for home health to assess patient needs, develop treatment plans, and deliver interventions that improve mobility and well-being.

Earn a $10K sign-on bonus for qualified full-time candidates. You will evaluate functional status, collaborate with physicians, educate patients and families, and document progress in accordance with state guidelines.

Qualifications

• Current Physical Therapy licensure in the state of Washington.
• Current CPR certification.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates the patient's functional status and PT needs.
• Assists physician in developing the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and plan of care.
• Observes and reports patient response to treatment and changes in condition.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs and informs health care team, patient, and family regarding plan of care and progress.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Develops PT plan of care within state guidelines.
