Job summary

Capital Region Home Health, part of the LHC Group and Optum family, seeks a licensed Physical Therapist to assess and plan therapy for home health patients, evaluating functional status and mobility to improve well-being.

You will implement plans of care, document progress, and collaborate with nurses and physicians to ensure care aligns with state guidelines. Full-time role includes a $10,000 sign-on bonus and comprehensive benefits.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists the physician in the plan of care.
• Provides services within the scope of practice as defined by state laws and the plan of care.
• Observes and reports patient response to treatment to the supervising nurse/physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patient, family, and health team about plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes all evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
