Job summary

Home Care Plus, part of LHC Group in the Optum family, seeks a Physical Therapist in Home Health to assess patient needs and determine treatment plans to improve function and mobility. You will collaborate with nurses and physicians to implement care plans across our Greenbrier, Pocahontas, and Monroe counties.

The role offers a competitive pay range with a sign-on bonus and relocation assistance, contingent on eligibility. Requires active PT licensure, CPR, and reliable transportation.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Provides services within the scope of practice as defined by state laws.
• Observes and reports the patient's response to treatment and changes in condition.
• T Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs and informs care team, patient, and family regarding plan of care.
