Job summary

CenterWell Home Health is seeking a licensed Physical Therapist to join our patient-focused care team. You will provide in-home therapy aimed at regaining strength, mobility and independence.

Reporting to the Clinical Manager, you will plan and implement treatment, document progress, and coordinate with physicians and care team to support optimal outcomes. You will also supervise PTAs and Home Health Aides, ensuring clear instructions and accurate documentation across the care team.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a healthcare team.

Responsibilities

• Plan and administer prescribed physical therapy treatment in patients' homes.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop and implement rehabilitation programs aligned with physician plans.
• Provide guidance to PTAs and Home Health Aides and supervise documentation.
