Job summary

CenterWell Home Health is seeking a licensed Physical Therapist to provide home-based therapy, focusing on restoring strength, mobility, and independence for patients in their own homes. You will work under the Clinical Manager and collaborate with a multidisciplinary team to develop and monitor individualized rehabilitation plans.

This role requires current PT licensure and CPR certification, with preferred experience in home health settings.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• One year of experience as a Physical Therapist (preferred).

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes to attain highest level of physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
• Utilize skills to positively impact patient outcomes.
