Job summary

Preferred Care Home Health is seeking a Physical Therapist Assistant (PTA) for Home Health PRN in Fort Myers, FL. Deliver skilled physical therapy under the direction of a licensed PT, guide patients through therapeutic exercises, document progress, and collaborate with the care team.

Eligible candidates are graduates of an accredited PTA program with an active Florida PTA license, CPR, valid driver's license, and strong communication and documentation skills; home health experience is a plus.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Active PTA license in the state of practice.
• Current CPR certification.
• Valid driver's license, reliable transportation.
• Home health, rehab, or skilled nursing experience a plus, not required.
• Strong communication, organizational, and documentation skills.

Responsibilities

• Deliver skilled physical therapy under the direction of a licensed PT and in line with the physician's plan of care.
• Guide patients through therapeutic exercises to build mobility, strength, balance, and function.
• Track and document patient progress toward treatment goals.
• Teach patients and caregivers safe techniques and home exercise programs.
• Complete clinical notes and progress reports accurately and on time.
• Collaborate with physicians, therapists, nurses, and caregivers as part of a coordinated care team.
• Take part in case conferences and care planning as needed.
