Job summary

Preferred Care Home Health in Fort Myers, FL is seeking a licensed Physical Therapist (PT) to join our Home Health team on a full-time basis.

You will evaluate patients, develop individualized treatment plans, provide skilled PT services to improve mobility and independence, monitor progress, communicate changes with physicians, educate patients and caregivers, and collaborate with the Director of Rehabilitation to ensure compassionate, high-quality care at home.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Florida Physical Therapist license in good standing.
• Minimum one year of physical therapy experience.
• Home health experience preferred but not required.
• Current CPR certification.
• Valid Florida driver's license, reliable transportation.
• Strong communication, time management, and patient education skills.

Responsibilities

• Evaluate patients and create individualized treatment plans based on physician orders.
• Provide skilled physical therapy services to improve strength, mobility, balance, endurance, and functional independence.
• Monitor patient progress and modify treatment plans as appropriate.
• Communicate changes in patient condition to the physician and interdisciplinary care team.
• Educating patients and caregivers on home exercise programs, safety techniques, adaptive equipment, and fall prevention.
• Supervising Physical Therapist Assistants (PTAs) in collaboration with the Director of Rehabilitation.
• Completing accurate and timely clinical documentation.
• Participating in case conferences, care coordination, quality improvement activities, and continuing education.
