Job summary

BAYADA Home Health Care is seeking a Physical Therapist to join the Sierra Vista Visits team. You will work with clients in their homes, craft treatment plans, and apply techniques to help clients reach goals as part of a multidisciplinary team delivering compassionate, reliable care.

Responsibilities include performing assessments, developing care plans, educating clients and families, and documenting care using an EMR system. Requires AZ PT license and program accreditation awareness.

Qualifications

• AZ Physical Therapist license required.
• Program accreditation per APA or APTA guidelines as indicated by transcript/diploma.
• Proficiency with EMR systems preferred.

Responsibilities

• Perform diagnostic tests and measurements in home health settings.
• Develop and implement individualized care plans including therapy exercises and gait training.
• Educate clients and families on rehabilitative care and safety at home.
• Document observations, interventions, and evaluations using EMR and tablets.
• Revise care plans based on ongoing assessments.
