Job summary

BAYADA Home Health Care is seeking a licensed Physical Therapist (PT) to join our Big Island Visits team. In this home‑based role, you will craft and implement individualized treatment plans, perform mobility and strength assessments, and work within a multidisciplinary team to help clients reach their goals.

You’ll educate clients and families, document care in an EMR, and adapt plans as needs evolve, with opportunities for comprehensive benefits and a supportive work environment.

Qualifications

• Must hold a current Hawaii Physical Therapist license.
• Graduation from an APTA-approved program, as evidenced by transcript or diploma.
• Solid computer skills; prior EMR experience preferred.

Responsibilities

• Develop and implement individualized PT care plans, including therapeutic exercises and gait training.
• Perform diagnostic tests and mobility assessments to guide treatment.
• Educate clients and families on rehabilitative care and activities for health, safety, and independence.
• Document observations, interventions, and evaluations in a state‑of‑the‑art EMR system.
• Collaborate with a multi‑disciplinary health care team delivering home health services.
• Maintain and revise PT care plans as client needs evolve.
