Job summary

Advocate Health at Home Cleveland - Home Care Physical Therapy is seeking a Part-time Physical Therapist to manage assigned patient caseload in the home setting. You will perform assessments, document results, and coordinate with physicians to ensure adherence to the plan of care.

Responsibilities include data collection, prioritizing patient needs, and leading a multidisciplinary team to promote safe, high-quality outcomes. Travel to patient homes is required as part of the role.

Qualifications

• Doctorate in Physical Therapy from an accredited school.
• Current state PT license and BLS certification.
• Strong data collection and documentation skills.

Responsibilities

• Manage daily patient caseload in home care with timely care and documentation.
• Assess health data from patient evaluations and EMR to plan care.
• Develop and implement plan of care with physician collaboration.
