Job summary

Lorian Health in Southwest Long Beach is seeking a Home Health PT to deliver patient-centered therapy in home settings. You will implement treatments, update care plans, and collaborate with physicians and case managers to optimize functional outcomes for clients.

The role requires a PT license, CPR, and 2+ years’ experience where possible, plus a reliable vehicle and a strong bedside manner. This position offers a sign-on bonus and a comprehensive benefits package to support patient care

Qualifications

• Graduate of a physical therapy school approved by the AMA Council of Medical Education or equivalent.
• Licensed to practice PT in the state of practice.
• Preferably 2+ years of PT experience.
• Maintains CPR certification and valid driver’s license.

Responsibilities

• Provide treatment as ordered by the attending physician.
• Develop and update Plan of Care (POC).
• Assist physician in evaluating level of function.
• Record and report patient conditions to physicians and case manager.
• Instruct family and other staff in therapy program.
• Supervise other healthcare personnel when appropriate.
• Evaluate home environment and make recommendations.
• Complete accurate clinical notes per policies.

As a Home Health PT, you will:

• Providing treatment as ordered by the attending physician, dentist, or podiatrist.
• Assisting in developing and updating the plan of treatment or Plan of Care (POC).
• Assisting the physician in evaluating level of function.
• Observing, recording, and reporting information on the patient’s condition to the attending physician, case manager, and in the patient’s health record.
• Advising, consulting, and when appropriate, instructing family and other LORIAN personnel in patient’s therapy program.
• Teaching and supervising other healthcare personnel when appropriate.
• Evaluating the home environment and making appropriate recommendations.
• Participating in in-service education programs.
• Completes, maintains, and submits accurate and relevant clinical notes regarding patients/clients condition and care given in accordance with LORIAN policies and procedures.

Qualifications

• Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in Association or its equivalent.
• Licensed or registered to practice as a PT in the state of practice.
• Preferably at least two (2) years of experience as a PT.
• Possesses and maintains good physical stamina and mental health. Presents a pre-employment physician’s health clearance, including negative TB skin tests and/or CXR and other tests as required by specific state statute.
• Has the ability to read and follow written instructions and document care given.
• Is self-directing and has the ability to work with little direct supervision.
• Possesses and maintains current CPR certification.
• Is flexible and cooperative in fulfilling the role obligations.
• Demonstrates tact, patience, and good personal hygiene.
• Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.
• Must be able to verbally communicate in the language of the client and document all client-related activity practices.

This position is based on a Pay per visit compensation model.

Pay per visit ranges from $100 to $150 visit commensurate with experience and depending on type of visit.
