Job summary

Lorian Health is seeking a licensed physical therapist to provide in-home care in the Modesto surrounding areas. The role requires evaluating patients, developing plans of care, and collaborating with physicians and families to improve function.

You will join a mission-driven team focused on delivering exceptional patient care, with a rotating weekend schedule and a compensation model based on pay per visit, reflecting experience and visit type.

Qualifications

• Minimum of one year of recent experience in a Home Health or Hospice Setting.
• Graduate of a physical therapy school approved by the Council of Medical Education of the AMA or its equivalent.
• Licensed or registered to practice as a PT in the state of practice.
• Ability to read and follow written instructions and document care.
• Self-directed with ability to work with little supervision.
• Current CPR certification.
• Licensed driver with insured vehicle per Lorian Health requirements.
• Ability to verbally communicate in the language of the client and document activities.

Responsibilities

• Providing treatment as ordered by the attending physician, dentist, or podiatrist.
• Assisting in developing and updating the plan of treatment (POC).
• Assisting the physician in evaluating level of function.
• Observing, recording, and reporting information on the patient’s condition to the attending physician, case manager, and health records.
• Advising, consulting, and instructing family and other LORIAN personnel in the patient’s therapy program.
• Teaching and supervising other healthcare personnel when appropriate.
• Evaluating the home environment and making recommendations.
• Participating in in-service education programs.

Responsibilities

• Assisting the physician in evaluating level of function.

• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
• Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in Association or its equivalent.
• Licensed or registered to practice as a PT in the state of practice.
• Has the ability to read and follow written instructions and document care given.
• Is self-directing and has the ability to work with little direct supervision.
• Possesses and maintains current CPR certification.
• Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.
• Must be able to verbally communicate in the language of the client and document all client-related activity practices.

This position is based on a pay per visit compensation model. Pay per visit ranges from $110 to $170 visit commensurate with experience and depending on type of visit.
