Job summary

Pinnacle Home Care, Florida’s largest independent Medicare-certified home health provider, seeks Physical Therapists to join a patient-centered team delivering high-quality care in patients’ homes.

Responsibilities include conducting assessments, creating and updating treatment plans, and coordinating with physicians and the care team to achieve optimal outcomes. A current PT license and BLS are required, with home health experience preferred.

Qualifications

• Must have a current PT license (state-specific).
• Graduate of an accredited physical therapy program.
• Valid BLS certification.
• Minimum one year of PT experience (home health experience preferred).

Responsibilities

• Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
• Develop and implement tailored treatment plans in consultation with physicians and clinicians, adjusting as needed.
• Collaborate with the interdisciplinary care team to optimize patient outcomes.
• Provide education and support to patients and families/caregivers on therapy programs.
• Maintain accurate clinical records and report any changes in the patient’s condition to the physician.
• Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.
• Participate in in-services, case conferences, and meetings to promote coordinated care with healthcare team members.
• Identify patient and family/caregiver needs for additional services and make appropriate referrals.
• Confirm daily visit schedules to ensure seamless coordination with other staff.
• Communicate patient progress and changes with physicians and care teams.
• Document all visits and progress notes on the same day of service.
