Job summary

Home Health Care, Inc. is seeking a Community-Based Physical Therapist to assess patients in their homes across the Twin Cities Metro Area and develop personalized treatment plans.

You will work to improve mobility, strength, balance, and functional independence while educating patients and caregivers to support long-term recovery. Collaboration with nurses and physicians is essential for optimal outcomes.

Qualifications

• Active Minnesota Physical Therapist (PT) license required.
• Strong clinical reasoning and communication skills.
• Passion for helping patients achieve meaningful outcomes.
• Ability to work independently in a field-based setting.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate patients in their homes and create individualized treatment plans.
• Improve mobility, strength, balance, endurance, and functional independence.
• Educate patients and caregivers to support long-term recovery.
• Collaborate with nurses, physicians, therapists and other team members.
• Monitor progress and adjust treatment plans to maximize outcomes.
• Complete accurate and timely documentation.
