Halcyon Home LLC in Manchaca, Texas seeks a Home Health Physical Therapist (Full Time) to serve the geriatric home health population in South Austin and surrounding areas. The role offers competitive pay and strong team support, plus generous PTO and holidays.

You will perform OASIS assessments, develop the plan of care with patients and families, educate on transfers and equipment, and coordinate with the care team while supervising assistants as needed. A current PT license is required.

Qualifications

• Current PT license is required.
• Experience in home health or related healthcare setting preferred.
• Excellent communication and interpersonal abilities.

Responsibilities

• Perform ongoing clinical assessment including OASIS to determine patient needs and function.
• Develop and evaluate the plan of care with patient and caregiver(s).
• Complete clinical notes and documentation in a timely manner.
• Educate patients and families on transfers and use of equipment.
• Coordinate with physicians and other health care providers.
• Supervise PT assistants and home health aides when applicable.
