Job summary

Halcyon Home LLC in Austin, TX is seeking a full-time Home Health Physical Therapist to support geriatric clients in North Austin and surrounding areas. You will perform OASIS assessments, develop and implement plans of care, and collaborate with patients, families, and caregivers to improve functional outcomes.

Qualifications

• Current PT license in good standing.
• Experience in home health or related setting preferred.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities.

Responsibilities

• Performs ongoing clinical assessment including OASIS to identify needs and level of functioning.
• Develops and evaluates plan of care with patient and caregiver(s).
• Instructs patients and families on transfer techniques and use of equipment.
• Provides counseling and education to patients and families.
• Documents and revises clinical notes and plan of care timely.
