Job summary

Halcyon Home LLC is seeking a dedicated Home Health Physical Therapist to serve South Austin and surrounding areas. This full-time role offers generous PTO and a supportive team in a growing geriatric home health program.

The incumbent will perform OASIS assessments, develop and adjust the care plan with patients and families, and coordinate with the health care team. Strong clinical skills and independent time management are essential.

Qualifications

• Current PT license.
• Experience in home health or related setting preferred.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage time effectively.

Responsibilities

• Conduct OASIS assessments and clinical evaluations at appropriate time points.
• Develop and evaluate the plan of care with patient and caregiver.
• Maintain required continuing education per agency and licensure board.
• Educate patients and families on transfer techniques and equipment safety.
• Document notes, orders, and adjust POC as needed.
• Collaborate with physicians and health care team on care plans.
• Supervise PT assistants and home health aides when applicable.
• Treat neuromuscular, neurological, and orthopedic conditions per referrals.
