BAYADA Home Health Care is seeking a Home Health Physical Therapist for a full-time, per-visit position in Upper Darby, PA. The PT will perform comprehensive home visits for adult and geriatric clients in the Mainline area, evaluating mobility, wound care, and developing individualized care plans.

Formación

• PA Physical Therapist license required.
• Minimum of 1 year of PT experience.
• Graduation from a PTA program approved by APTA/AMA.
• Ability to work independently and manage time effectively.

Responsabilidades

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility, gait, balance, and strength.
• Provide wound care and protective interventions using evidence-based dressings.
• Develop and implement individualized care plans including exercises and training.
• Educate clients and families on rehabilitative care and activities.
• Accurately document observations and interventions in EMR.
