Job summary

Responsibilities

• Performs evaluations, determines PT diagnoses, develops plan of care, and provides PT procedures to restore function and mobility.
• Conducts timely reassessments and OASIS assessments as appropriate and documents outcomes.
• Updates plan of care in collaboration with physicians and interdisciplinary team members.
• Supervises assistants and aides as needed and completes supervision documentation.
• Participates in team meetings and supports training of staff and students.
• Prepares progress notes and discharge plans to ensure continuous care.
