Job summary

PARS Therapy is seeking a compassionate and skilled Physical Therapist to join our home health team in Devine, TX. The PT will evaluate patients, develop treatment plans, and deliver evidence-based care in patients' homes.

Requirements include an active Texas PT license, graduate degree in Physical Therapy, and experience in home health or rehabilitative settings. Excellent communication and patient education are essential.

Qualifications

• Evaluates patients and functional limitations in a home health setting.
• Develops and implements individualized treatment plans.
• Provides exercise, gait training, manual therapy and balance retraining.
• Educates patients and caregivers on home programs and safety.
• Monitors progress and adjusts plans for outcomes.
• Documents evaluations and sessions using digital platforms.
• Collaborates with physicians, nurses and therapists.
• Performs home safety assessments to reduce falls.
• Delivers compassionate, patient-centered care.

Responsibilities
