Job summary

Freedom at Home is seeking a Physical Therapist to join our home health team, delivering skilled PT services in patients’ homes within Michigan. The role emphasizes compassionate care, independence, and work-life balance, with a sign-on bonus and per-visit compensation.

Responsibilities include evaluating patients, developing treatment plans, educating patients and caregivers, and ensuring timely documentation. A Michigan PT license, CPR, and at least 1 year of PT experience are required.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Michigan Physical Therapist license.
• Current CPR certification.
• Minimum of 1 year of PT experience.
• Home Health experience is a plus but not required.
• Valid driver's license, reliable transportation, and auto insurance.

Responsibilities

• Evaluate patients in their homes and develop individualized treatment plans.
• Provide skilled physical therapy services.
• Educate patients and caregivers on exercises, safety, and mobility.
• Complete OASIS and clinical documentation in a timely manner.
• Collaborate with physicians, nurses, and other healthcare professionals.
• Monitor patient progress and adjust treatment plans as needed.
• Promote safe, high-quality patient care and excellent outcomes.
