Advanced Home Health & Hospice Inc in Grass Valley, CA is seeking a Physical Therapist to join our Home Health Team. You may work full-time or per diem and will provide skilled PT services under physician plans of care.

The role requires licensing in California, CPR certification, and typically two years of home health experience, with duties including patient assessment, treatment direction, caregiver instruction, discharge planning, and coordination with the clinical team.

Qualifications

• Possesses a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization.
• Licensed to practice as physical therapist within the State, if applicable.
• Two years of appropriate experience as a physical therapist. Community/home health experienced preferred.

Responsibilities

• Provides physical therapy services to patients per physician’s plan of care.
• Assesses therapeutic status and develops the total plan of care.
• Directs therapy treatment and trains patients/families in usage of therapeutic devices.
• Reports patient progress and participates in discharge planning.
• Supervises PT Assistants and evaluates aides when therapy is sole skilled service.
