Job summary

AdvisaCare in Michigan is seeking a dedicated Physical Therapist to improve mobility and independence for clients in home health settings.

You will evaluate patients, develop personalized treatment plans, and deliver hands-on therapy while collaborating with an interdisciplinary team and educating families on safety and rehabilitation goals.

Qualifications

• Must hold a valid Physical Therapist license in Michigan.
• Prefer at least 1 year of home health care experience.
• Strong communication and teamwork skills.
• Demonstrated compassion and patient-centered care.
• Ability to work independently and manage a schedule.

Responsibilities

• Develop and implement individualized treatment plans to improve mobility, strength, balance, and function.
• Perform therapeutic exercises and interventions to help clients regain independence and reduce pain.
• Assess and document patient progress, adjusting plans as needed.
• Collaborate with interdisciplinary care teams to ensure continuity of care and outcomes.
• Educate patients and families on exercises, safety, and injury prevention.
• Maintain accurate documentation and comply with professional standards.
