Job summary

Premier Home Health Providers Inc. seeks a per diem Physical Therapist to deliver skilled home health PT services across California. You will evaluate mobility, design individualized plans, and educate patients and families to promote recovery in the home setting.

Responsibilities include coordinating with physicians, supervising aides when needed, and ensuring accurate documentation. Flexible schedule and potential conversion to full-time based on performance are offered.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Current CPR certification with AHA.
• Valid driver’s license and reliable, insured vehicle.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.
• Oversee Home Health Aide care bi-weekly when PT is the sole skilled service.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.
