Job summary

Qualifications

• Active Physical Therapist license in Texas.
• Professional liability insurance coverage.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Home health or rehabilitative setting experience preferred.

• Up-to-date knowledge of evidence-based treatment techniques.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Monitor progress and adjust plans to achieve optimal outcomes.
• Document evaluations, treatments, and progress notes in digital platforms.
• Collaborate with physicians, nurses, and occupational therapists.
• Perform home safety assessments and recommend fall-prevention strategies.
• Deliver compassionate care to empower patients toward independence.
