Job summary

PARS Therapy in Kingwood, TX is seeking a compassionate Physical Therapist (PT) to join our home health team. You will evaluate patients at their homes, develop treatment plans to restore function, and provide interventions including exercise, gait training, manual therapy, and balance retraining.

The role emphasizes patient-centered care, collaboration with physicians, nurses and therapists, home safety assessments, and educating patients and caregivers.

Qualifications

• Active Physical Therapist license in Texas.
• Professional liability insurance coverage for Texas license.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care approaches to individual patients.
• Knowledge of evidence-based treatment techniques and best practices.
• Excellent communication and professional demeanor with patients.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized plans to restore function, mobility, and reduce pain.
• Provide interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies.
• Monitor patient progress and adjust plans to achieve optimal outcomes.
• Document evaluations, treatments, and progress notes using digital platforms.
• Collaborate with physicians, nurses, and other therapists on the care plan.
• Conduct home safety assessments and recommend fall-prevention measures.
• Deliver compassionate care while promoting patient independence.
