Job summary

Pars-Therapy-1 is seeking a licensed Physical Therapist to join a forward-thinking home health team in Texas. You will evaluate patients in home settings, develop individualized treatment plans, and deliver hands-on interventions focused on restoring function and reducing pain.

Qualifications

• Active Physical Therapist license in TX required.
• Professional liability insurance coverage for TX license required.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Ability to adapt care approaches to diverse patient needs.
• Up-to-date knowledge of evidence-based treatment techniques.
• Excellent communication skills and professional demeanor with patients.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function and reduce pain.
• Provide interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Monitor progress and adjust plans to achieve optimal outcomes.
• Accurately document evaluations, treatments, and progress using digital platforms.
• Collaborate with physicians, nurses, and other therapists as part of the care team.
• Perform home safety assessments and recommend measures to prevent falls or re-injury.
• Deliver compassionate care while promoting patient independence.
