Job summary

Pars-Therapy-1 is seeking a licensed Physical Therapist to join our Tyler, TX team. You will evaluate patients in home health settings and implement individualized plans to restore function, mobility and reduce pain.

You will use modern digital platforms to document progress, educate patients and collaborate with an interdisciplinary team, all while promoting compassionate, patient-centered care.

Qualifications

• Active Physical Therapist license in Texas (TX).
• Professional liability insurance coverage for TX PT.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Experience in home health or rehabilitative settings preferred.
• Strong ability to adapt care approaches to patient needs.
• Up-to-date knowledge of evidence-based treatment techniques.
• Excellent communication and professional demeanor with patients.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in home health settings.
• Develop and implement individualized treatment plans to restore function and mobility.
• Provide therapeutic interventions including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Monitor patient progress and adjust plans to achieve optimal outcomes.
• Accurately document evaluations and progress using digital platforms.
