Job summary

Concierge Home Care is hiring a Home Health Physical Therapist (PT) to deliver one-to-one in-home care in the Tallahassee area. This role offers flexible scheduling, autonomy, and the opportunity to boost earnings through productivity and bonuses.

You will evaluate patients, create individualized treatment plans, and work with a supportive team while enjoying modern EMR tools and robust onboarding. Employment supports full-time, modified full-time, PRN, or 1099 options.

Qualifications

• Active, unencumbered Florida Physical Therapist license.
• Valid driver’s license and reliable transportation are required.
• Auto insurance is recommended for transportation to patient homes.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on safety and recovery.
• Collaborate with physicians and interdisciplinary teams.
• Supervise Physical Therapist Assistants (PTAs).
• Complete documentation using streamlined technology.
