Use AI-assisted charting and WellSky/Kinnser EMR to minimize documentation time while delivering 1:1 patient care in the home. You will evaluate patients, create individualized plans, and collaborate with physicians.

Qualifications

• Active Florida Physical Therapist license is required.
• Valid driver’s license and reliable transportation are required.
• Home Health PT role with flexible schedule requires licensure and compliance.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on safety and recovery.
• Collaborate with physicians and interdisciplinary teams.
• Supervise Physical Therapist Assistants (PTAs) and complete documentation using the EMR.
