Job summary

Concierge Home Care is seeking a Home Health Physical Therapist (PT) in Lakeland, FL with a flexible schedule and autonomy to manage patient care in the home. This full-time role offers a pay-per-visit structure with earning incentives and a potential average full-time earnings of around $120,000+ per year.

In this position, you will evaluate patients, develop individualized treatment plans, provide therapeutic exercises, and educate patients and caregivers on safety and recovery.

Qualifications

• Active, unencumbered Florida Physical Therapist license.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on safety and recovery.
• Collaborate with physicians and interdisciplinary teams.
• Supervise Physical Therapist Assistants (PTAs).
• Complete documentation using streamlined technology.
