Job summary

Careage Home Health is seeking a Physical Therapist to join our team and grow PT skills while delivering essential one-on-one home care. Travel to multiple locations in the Renton area with a competitive hourly rate for patient visits, charting, and travel time.

Qualifications

• Graduate from a physical therapist curriculum approved by the APTA.
• Currently licensed as a physical therapist in Washington state.
• Two years' experience preferred.
• Home Health experience preferred.
• Valid WA driver's license.

Responsibilities

• Provide physician orders and plan of care under the direction of the Director of Clinical Services.
• Collaborate with home care personnel in patient care planning.
• Direct and supervise Physical Therapy Assistant and Home Health Aide as assigned.
• Take initial patient history and perform initial evaluation.
• Consult with physicians regarding changes in treatment.
• Instruct patients and family in home programs and activities of daily living.
• Participate in QA/PI activities as assigned.
• Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition.
• Appropriately utilize ICD-10 codes.
