Job summary

Penn Medicine Home Health Lancaster is seeking a compassionate Physical Therapist to join our home-based care team. You will evaluate patients, design individualized treatment plans, and deliver skilled PT services in patients' homes to restore function and mobility.

Qualifications

Licensed Physical Therapist in Pennsylvania.
Graduate of a CAPTE-accredited Physical Therapy program.
BLS/CPR Certification.
Valid Driver's License and auto insurance.

Responsibilities

Evaluate patients and develop individualized treatment plans.
Provide skilled physical therapy services in the home setting.
Restore function, improve mobility, reduce pain, and prevent disability.
Collaborate with interdisciplinary healthcare teams to optimize patient outcomes.
Empower patients and families through education and support.
