Qualifications

• Must have a current PT license (state-specific).
• Minimum one year of PT experience (home health experience preferred).
• Must have a valid driver’s license and access to a reliable and insured vehicle.

Responsibilities

• Perform initial and ongoing patient assessments including OASIS evaluation.
• Develop and implement tailored treatment plans in consultation with physicians.
• Maintain accurate clinical records and report any changes in patient’s condition.
