Job summary

Pinnacle Home Care in Lake City, Florida, is actively seeking Full-Time & PRN Physical Therapists (PTs) to join our dedicated team. You will be responsible for providing exceptional patient care in the home environment, performing assessments, and collaborating with healthcare professionals to optimize treatment plans.

Ideal candidates will have a current PT license, graduate from an accredited program, and excellent communication and organizational skills. The position offers flexible scheduling and a supportive work culture dedicated to professional development.

Qualifications

• Must have a current PT license (state-specific).
• Graduate of an accredited physical therapy program.
• Valid BLS certification.
• Minimum one year of PT experience (home health experience preferred).
• Excellent communication, organizational, and time-management skills.
• Must have a valid driver’s license and access to a reliable vehicle.

Responsibilities

• Perform initial and ongoing patient assessments, including OASIS evaluation.
• Develop and implement tailored treatment plans in consultation with physicians.
• Collaborate with the interdisciplinary care team to optimize outcomes.
• Educate and support patients and families/caregivers on therapy programs.
• Supervise Physical Therapy Assistants as per agency policies.
