Qualifications

• Arizona PT license in good standing.
• Graduate of an APTA-approved PT program.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Evaluate and treat clients to improve mobility and function.
• Develop and implement individualized care plans.
• Document observations and interventions in the EMR.
• Educate clients and families on home-based rehab.
• Collaborate with interdisciplinary team to support goals.
