Job summary

Concierge Home Care is hiring a Home Health Physical Therapist (PT) to provide 1:1 in-home care in Apopka, FL and surrounding areas. Enjoy a flexible schedule, autonomy, and the potential to increase income through productivity.

You will evaluate patients, develop individualized treatment plans, administer therapeutic exercises, educate families, and collaborate with physicians; documentation is streamlined with AI charting and WellSky/Kinnser.

Qualifications

• Active, unencumbered Florida Physical Therapist license.
• Lymphedema Certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on safety and recovery.
• Collaborate with physicians and interdisciplinary teams.
• Supervise PTAs.
• Complete documentation using streamlined technology.

Florida Physical Therapist license (Active)
