Job summary

BAYADA Home Health Care is seeking a Home Health Physical Therapist to serve patients across Bucks County, PA, based in Bristol, PA. This is a full-time position paid per point with a $5,000 sign-on bonus for eligible staff.

You’ll provide skilled PT care in patients’ homes, perform mobility and balance assessments, and develop individualized plans while collaborating with the multidisciplinary team for safe, quality home health care.

Qualifications

• Active and current license to practice as a professional Physical Therapist (PT) in PA
• Ability to travel to cases as assigned
• Strong written and verbal communication and compassionate patient care
• Develop and revise care plans with multidisciplinary team

Responsibilities

• Treat patients in their home as the therapist of record.
• Perform diagnostic tests and mobility assessments.
• Develop and implement individualized care plans including exercises.
• Collaborate with nursing and other team members on the care plan
