Job summary

BAYADA Home Health Care, Inc. is seeking a Home Health Physical Therapist to provide compassionate in-home therapy in Florence, New Jersey. This full-time, staff position offers a $5,000 sign-on bonus and pay-per-point structure in line with experience.

You will perform PT duties in patients' homes, assess mobility and safety, develop care plans, and coordinate with the clinical team to ensure high-quality care. A PA license and willingness to travel are required.

Qualifications

• Active and current Physical Therapist license in PA
• Willingness to travel to patient homes as needed
• Excellent written and verbal communication with patients and families

Responsibilities

• Provide in-home physical therapy services to patients
• Assess mobility, balance, strength, and gait; develop individualized care plans
• Collaborate with care teams and nursing to optimize patient outcomes
• Document care plans and progress appropriately
