Interim HealthCare Midatlantic is seeking a Home Health Physical Therapist to deliver personalized therapy at patients' homes in Carbon County, PA. You will have clinical autonomy to evaluate, set goals, and adjust treatment within the plan of care, with a supportive, responsive team backing you up.

The role offers competitive per-visit pay ($65-$125), weekly payment based on experience and visit type, and opportunities for online training, continuing education, and tuition discount programs.

Qualifications

• Graduate of a CAPTE-accredited physical therapy program with a current Pennsylvania PT license.
• Two years of physical therapy experience relevant to the patient population within the past five years preferred.
• Home health experience preferred.
• Current CPR certification.
• Valid driver's license, auto insurance, and reliable transportation.
• Strong clinical judgment, communication, organization, and documentation skills.

Responsibilities

• Evaluate function, mobility, strength, balance, safety, and equipment needs in the home.
• Help develop the plan of care with measurable short- and long-term goals.
• Provide therapeutic exercise, gait and transfer training, and other interventions per the plan of care.
• Reassess at each visit, complete re-evaluations, and obtain revised orders when indicated.
• Educate patients, families, and caregivers on exercises, fall prevention, and safe mobility.
• Supervise physical therapist assistants as assigned.
• Document treatment, response, and progress accurately and on time, and support discharge planning.
