Job summary

DFW Home Health, part of LHC Group, seeks a Physical Therapist to perform patient assessments, plans of care, and therapeutic interventions in a home health setting. You will evaluate mobility, pain, endurance, and functional status to guide effective treatment and progress reporting.

This role offers a combination of per-visit point compensation and hourly pay with a comprehensive benefits package. Candidates must hold PT licensure in the practicing state and have access to reliable

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification (or ability to obtain within 90 days of hire).

Responsibilities

• Evaluates patient functional status and PT needs and assists in developing the therapy plan of care.
• Provides PT services within scope of practice and in coordination with the care team.
• Monitors patient response to treatment and reports changes to supervisor/physician.
• Treats patients to relieve pain, improve function, and maintain maximum functioning.
• Educates patient and family about plan of care, limitations, and progress.
