Job summary

DFW Home Health, part of the LHC Group, is hiring a Full-Time Physical Therapist / DPT to serve Weatherford, TX, with coverage in Springtown and Azle. The role offers a $10,000 bonus for full-time employment and relocation assistance.

You will assess functional status, develop and implement therapy plans, and collaborate with a multidisciplinary team to promote patient well-being in the home setting. The position emphasizes compassionate, patient-centered care with opportunities for growth

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and access to a dependable vehicle or public transportation.
• Ability to work in home health across Weatherford, Springtown, and Azle.

Responsibilities

• Evaluates patient functional status and PT needs; assists physician in therapy plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient response to treatment to supervising nurse/physician.
• Treats to relieve pain, restore function, maintain maximum functioning.
• Instructs patient and family/caregivers about plan of care and progress.
• Visits patients according to Plan of Treatment; completes progress notes per policy.
• Completes evaluations and develops PT plan of care within state guidelines.
• Reports outcomes, goals, and anticipated frequency of care.
